We know patients and family members often have many questions before coming to Maple Manor. There is no need to worry! We will facilitate a smooth admission at every step of the way.
A comprehensive assessment will be completed by our Nursing Department. Our doctor and attending nurse will develop a plan of care which includes skilled nursing, subacute care and medication management, delivered by a highly qualified nurses and certified nurse aides. (RNs, LPNs, and CNAs)
A comprehensive assessment will be completed by our Therapy Department. For each patient, we specially design a progressive therapy plan and we designate a team of therapists for each patient. During your stay, you will be treated by the same therapists to maximize the consistency and continuity of care. To optimize our results, we only use our own in-house therapists. (We don’t use staffing companies).
Most patients will receive intense therapy 5 to 7 days a week. But this is adjusted depending on the patient’s ability and tolerance levels. Therapy takes place in our state-of-art gym or in the patient’s room depending on medical need. Maple Manor has an open door policy and we allow and encourage family to be involved in all therapy sessions.
Patients will meet the Dietary Manager, Dietary Aide and Registered Dietician. Together they will develop a menu specific to the patient’s preferences and medical restrictions. The dietary department can accommodate specific cultural, ethnic and religious diets. (e.g. Halal, Kosher) Gourmet meals and appetizing snacks are made with fresh ingredients. We take great pride in serving delicious and comforting food in a restaurant style atmosphere.
Maple Manor has an on-site social worker to assist the patient and family during the entire stay at Maple Manor. Once admitted, the Social Worker will greet the patient and family and complete a support assessment. This assessment identifies the patient’s current support system, family dynamics, home environment and community resources. Our initial care conference is scheduled within approximately 1 week of admission. Care conferences consist of the patient, family and our treatment team to discuss the progress, goals, and discharge. We strongly encourage the family to attend all care conferences and we believe in constant communication to keep everyone informed. Upon discharge, the patient and family can expect a smooth transition back home or to another medical facility.
